PROCEDURES FOR PLACEMENT OF A STUDENT INTO THE HOMEBOUND PROGRAM
1. The student must be registered and have a schedule built into the TEAMS system at the home campus
prior to holding a 504 or ARD meeting to dicuss possible Homebound Placement (NO EXCEPTIONS).
2. The parent's of the student must obtain the Physcian's statement from the school nurse and then have a
Physician complete the statement. Then the Physcian's Statement must be turned into the School nurse
at the student's home campus. The School nurse will notify the campus 504 administrator or ARD
adminstrator if the student is a Special Education student of the Physcian's statement.
3. The school nurse will fax the Physician's Statement to the Home/Hospital School (915) 231-5497.
4. The campus 504 administrator needs to begin the 504 referral process and gather all required
information for General Education Students. Grades for the student must be entered into TEAMS
before the 504 meeting is to take place (The student must have grades. NG for no grades or blank
grade books are unacceptable). When the campus 504 administrator is ready and prepared, including
grades entered into TEAMS, for a 504 meeting for possible Homebound placement please call the
Homebound school at (915) 231-5486 or (915) 231 5488 to set up a 504 meeting for general education
homebound students ( a 504 meeting is required for all general education students). The home campus
must provide the personale rquired for the 504 meeting. The Homebound Representative is not a
teacher on your campus and is not to be used as such.
5. The home campus will provide the required textbooks for the student.
6. General Education Homebound 504 Meeting:
The 504 committee must be made up of the parent, school administrator, regular education teacher
(from the home campus), school counselor, and a homebound representative. The meeting must be
intiated by the home campus. The home campus must contact the parent and the Homebound school to
set up the 504 meeting.

- The home campus must provide the homebound representative with a copy of the student's current schedule, current grades, student demographic, graduation plan, course audit sheet, and a transcript of completed course work, a copy of the 504 paperwork, and a copy of the student's drop form.
- If the placement is made, then the parent must complete the enrollment forms for the Homebound School.
- If the student is placed into the Homebound School the school administrator must then assist the parent in dropping the student from the home campus and provide the homebound representative with a copy of the drop form.
- An ARD must be held by the home campus. The home campus must contact the parents and the Homebound school to set up the ARD. The home campus is responsible for holding the meeting and and inviting the personale needed for the ARD. A class schedule must be guilt and grades must be entered into TEAMS before the ARD ( NG no grades or blank gradebooks are unacceptable). A completed Physicain's statement must be obtained from tne School nurse.
- It must be a fully constituted ARD made up of the parent, administrator, diagnostician or school psychologist, a regular education teacher (from the home campus), a special education teacher (from the home campus), related services personale, councelor, and a homebound representative. The homebound Representative is not to be used as a regular or special education teacher for your campus ARD.
- The home campus needs to provide the homebound representative with a copy of the student's current schedule, current grades, student demographic, graduation plan, course audit sheet, a transcript of completed course work, and the state special education folder containing a copy of the placement ARD.
- If the placement is made then the parent must complete the enrollment forms for the Homebound School.
- If the student is placed into the Homebound School the school administrator must then assist the parent in dropping the student from the home campus and provide the homebound representative with a copy of the drop form.
