THE PURPOSE OF THE HOME/HOSPITAL SCHOOL:
The purpose of the Home/ Hospital School is to provide an education to students in a home or hospital setting who are medically unable to attend school. The Texas Edcuation Agencies Student Accounting Handbook for 2012-2013 states that in order for a child to recieve homebound services the student must be confined at home or hospital bedside for a minimum of four weeks. They must be confined at home or hospital beside for medical reasons only. The student's condition is documented by a physcian licensed to practice in the United States. The Physcian's statement is a recomendation not the sole determing factor for placement. If you child is in the hospital the hospital staff will notify the Home/Hospital School.
PROCEDURES FOR THE PARENT:
1. If your childs is medically unable to attend school and is confined to the home go to the school nusre at you home
campus to obtain a Physcian's Statement.
2. Take the Physican's Statement to your doctor and they must complete the statement if they beleive that your child is
medically unable to attend school and is confined to the home for a period of four weeks or more.
3. You must then take the Physican's Statement back to the school nurse at your child's home campus.
4. You then need to complete the Section 504 Notice and Consent for Referral and Evaluation with the student's home
campus 504 adminstrator. The Home campus will then inform the Home/Hospital School of the 504 meeting to dicuss
Homebound placement. If your child is a Special Education student the an ARD will be held.

RULES AND ATTENDANCE POLICY FOR HOMEBOUND STUDENTS.
1. If your childs is placed into the homebound program the parent must complete the enrollment packet and drop the
student from their home campus after the 504 or ARD meeting.
2. Your child must be confined to the home for medical reasons in order to qualify. They may only leave the home for
medical appointments only.
3. There must be an adult (21 years of age or older) in the home while the Homebound Teacher is in your home.
4. The textbooks must be issued from the child's home campus and need to be in the home.
5. The student must be ready for class at the scheduled day and time.
6. The student is resposible for completing their assignments and turning them in when they are due.
7. If the child is too ill for the lesson, the parent must call the Home/Hospital school by 8:00 am at 231-5486 or 231-5488.
8. The student is expected to be in attendance at least 90% of the days while the schools are in secession. A student who attends at least 75% but less then 90% of the days school are in secession the student must complete a plan approved by the pricipal to recieve credit.
